ピコレーザーやM22は、シミ治療に用いられる代表的な医療機器であり、それぞれ異なる作用メカニズムを持っています。ピコレーザーで取れなかったシミに対して、M22で改善できる可能性はありますが、その効果はシミの種類や深さ、治療対象の状態によって異なります。以下に、ピコレーザーとM22の違い、およびM22がピコレーザーで改善しきれなかったシミに対してどのように有効かを詳しく説明します。
1. ピコレーザーの特徴と限界

Hello, I’m Koji Kato, a cosmetic dermatologist at A&O Clinic Osaka Nakatsu.
In this session, I will explain whether M22 can be effective for pigmentation that couldn’t be removed by the Pico Laser.
Pico Laser and M22 are both well-known devices used for treating pigmentation, each utilizing different mechanisms. While Pico Laser is highly effective for specific types of pigmentation, M22 may provide additional benefits in cases where Pico Laser alone isn’t sufficient. The effectiveness of M22 depends on the type and depth of the pigmentation being treated. Below, I’ll outline the differences between Pico Laser and M22 and explain how M22 may work for pigmentation that Pico Laser couldn’t fully address.
1. Characteristics and Limitations of Pico Laser
Pico Laser uses ultra-short pulses (picoseconds) to target melanin in the skin, effectively breaking down pigmentation. Due to the precision of the laser, it minimizes damage to the surrounding tissue while effectively targeting pigment-related issues such as dark spots and tattoos.
Pico Laser’s Effectiveness:
Highly effective for targeting melanin and treating dark spots, including superficial pigmentation.
Works well for pigmentation disorders such as melasma and deeper pigmented lesions.
Limitations:
May not be as effective for deeper pigmentation or more diffuse pigmentation spots.
Pico Laser’s ultra-short pulses sometimes cannot reach deeper layers of the skin or effectively target lighter pigmentation.
2. Characteristics of M22
M22 is a multifunctional platform that combines IPL (Intense Pulsed Light) and laser technology to treat pigmentation and improve skin texture. It offers the versatility to address not only pigmentation but also vascular issues and overall skin quality.
Modes of M22:
IPL (Intense Pulsed Light): Targets a broad range of pigmentation and vascular issues using various wavelengths of light.
Nd
Laser: Treats deeper pigmentation and vascular lesions that IPL alone may not address.
M22’s Effectiveness:
IPL treats superficial pigmentation and vascular lesions, which Pico Laser may not effectively target.
Nd
laser penetrates deeper into the skin to treat pigment in the deeper layers, complementing IPL and Pico Laser in cases of stubborn pigmentation.
M22 also promotes collagen production, improving overall skin texture while treating pigmentation.
3. How M22 Can Be Effective for Pigmentation Pico Laser Could Not Address
Different Mechanism of Action:
While Pico Laser targets melanin with short pulses, M22’s IPL delivers broader wavelengths, making it more effective for treating diffuse pigmentation and blood vessels that may contribute to pigmentation.
The combination of IPL and Nd
laser in M22 allows for treatment of both superficial and deeper pigmentation that Pico Laser may struggle to fully resolve.
Different Targets:
M22’s IPL not only targets melanin but also works on vascular-related pigmentation. Therefore, for pigmentation that Pico Laser was unable to treat, M22 may offer additional effectiveness by addressing these different targets.
4. Types of Pigmentation Best Treated with M22
M22 can be especially effective for the following types of pigmentation:
Diffuse, lighter pigmentation (such as melasma and age spots).
Pigmentation located in deeper layers of the skin.
Vascular pigmentation and redness.
When previous laser treatments (including Pico Laser) were not fully effective.
5. Combining Pico Laser and M22
While Pico Laser is excellent for targeting specific, dark pigmentation, M22 can address broader pigmentation and vascular issues that may not have fully responded to Pico Laser. Therefore, combining both treatments, or following up with M22 after Pico Laser, may provide more comprehensive results.
