AGAに対する内服治療のそれぞれの特徴について教えて、大坂の美容皮膚科医が解説

今回は、AGAに対する内服治療のそれぞれの特徴について解説します。
AGA（男性型脱毛症）に対する内服治療にはいくつかの選択肢があり、それぞれの治療薬には異なる特徴やメカニズム、効果が存在します。以下に、主な内服治療薬についての特徴を解説します。
1. フィナステリド（Finasteride）
商品名: プロペシア（Propecia）、フィナロイドなど
特徴:
フィナステリドは、5α-リダクターゼという酵素を抑制することで、テストステロンがジヒドロテストステロン（DHT）に変換されるのを防ぎます。DHTは、毛髪の成長サイクルを短縮し、毛包を萎縮させて脱毛を引き起こす原因物質です。
効果:
脱毛の進行を遅らせる: DHTの生成を抑制することで、脱毛の進行を抑える効果があります。
発毛促進: 一部の患者には、毛髪の再生や薄毛の改善が見られることがありますが、主な効果は脱毛の抑制です。
使用方法:
通常は、1日1回1mgを経口摂取します。
副作用:
性機能障害（リビドー低下、勃起不全など）が稀に報告されていますが、これは一時的であることが多いです。
乳房痛や肝機能障害などの副作用も稀に報告されています。
効果の発現時期:
効果が実感できるまでには、通常3〜6か月の使用が必要です。
2. デュタステリド（Dutasteride）
商品名: ザガーロ（Zagallo）、アボルブ（Avodart）など
特徴:
デュタステリドもフィナステリドと同様に5α-リダクターゼを抑制しますが、フィナステリドが主に「2型5α-リダクターゼ」のみを抑制するのに対し、デュタステリドは「1型」と「2型」の両方を抑制する点で異なります。このため、より強力なDHT抑制効果が期待されます。
効果:
フィナステリドよりも強力な脱毛抑制効果が期待されています。
脱毛抑制効果がより強い一方で、副作用のリスクも若干高いとされています。
使用方法:
通常、1日1回0.5mgを経口摂取します。
副作用:
フィナステリドと同様に、性機能障害や乳房痛、肝機能障害などが報告されています。
フィナステリドよりも副作用のリスクが高い可能性がありますが、個人差が大きいです。
効果の発現時期:
効果が実感できるまでには、通常3〜6か月の使用が必要です。フィナステリドよりも早い段階で効果が出ることがあるとされています。
3. ミノキシジル（Minoxidil）（内服版）
商品名: ロニテン（Loniten）など
特徴:
ミノキシジルはもともと血圧を下げる薬として開発されましたが、毛髪の成長を促進する副作用が発見され、現在はAGA治療に用いられています。血管拡張作用があり、頭皮の血流を改善することで、毛髪の成長を促進すると考えられています。外用薬が主流ですが、内服薬としても使用されることがあります。
効果:
発毛促進: 内服ミノキシジルは、外用ミノキシジルに比べて全身に作用するため、より強い発毛効果が期待されます。毛包の成長を促進し、毛髪の密度が改善されます。
早い段階での発毛効果: 通常、3〜6か月で発毛効果が実感できることが多いです。
使用方法:
用量は患者の状態によりますが、1日1回2.5〜5mgを服用します。
副作用:
血圧の低下、めまい、動悸など、もともとの血圧降下作用による副作用があります。
体毛が濃くなる（多毛症）ことがありますが、これも個人差があります。
浮腫や体重増加などが報告されることもあります。
効果の発現時期:
使用開始から2〜3か月で発毛が見られ、6か月ほどで目立った効果が出ることが多いです。
4. その他の内服治療薬
スピロノラクトン（Spironolactone）
特徴: 利尿薬として使われるスピロノラクトンは、男性ホルモン（アンドロゲン）を抑制する作用があるため、AGA治療に使用されることがあります。
効果: 男性ホルモンの働きを抑制することで、DHTによる毛髪への影響を軽減し、脱毛の進行を抑える効果が期待されます。
副作用: ホルモンバランスに影響を与えるため、性機能障害や乳房の腫れ、電解質異常が報告されています。
フルタミド（Flutamide）
特徴: 前立腺癌治療に使われる薬で、アンドロゲン受容体阻害剤として作用します。男性ホルモンが受容体に結合するのを阻害することで、脱毛を防ぐ効果があります。
効果: 脱毛抑制効果が期待されますが、他の治療法に比べて使用頻度は低いです。
副作用: 性機能障害、肝機能障害などが報告されています。
まとめ
AGAに対する内服治療は、主にホルモンや血流に働きかけるものが中心です。フィナステリドやデュタステリドは、男性ホルモンであるDHTの抑制に特化しており、特に脱毛抑制に優れています。一方、ミノキシジルは血流改善によって発毛を促進し、より直接的に毛髪の成長をサポートします。

Today, I will explain the different oral treatment options for AGA (Androgenetic Alopecia), focusing on their features and effects.
Several oral medications are available for AGA treatment, each with distinct mechanisms of action, characteristics, and results. Below, I will describe the main oral treatments used for AGA.
1. Finasteride
Brand names: Propecia, Finaroid, etc.
Features: Finasteride inhibits the enzyme 5-alpha-reductase, which converts testosterone into dihydrotestosterone (DHT). DHT is known to shorten the hair growth cycle and shrink hair follicles, leading to hair loss.
Effects:
Slows hair loss: By inhibiting DHT production, Finasteride helps slow down hair loss.
Promotes regrowth: In some cases, Finasteride can lead to hair regrowth or an improvement in thinning areas, although its primary effect is on slowing hair loss.
Usage: Typically taken orally, 1 mg once daily.
Side effects: Rare cases of sexual dysfunction (reduced libido, erectile dysfunction), breast tenderness, and liver function issues. These side effects are generally temporary.
Time to effectiveness: Noticeable results typically take about 3 to 6 months of consistent use.
2. Dutasteride
Brand names: Zagallo, Avodart, etc.
Features: Like Finasteride, Dutasteride inhibits 5-alpha-reductase, but it blocks both Type 1 and Type 2 5-alpha-reductase enzymes, making it a more potent inhibitor of DHT production.
Effects:
More potent DHT suppression: Dutasteride is generally considered stronger than Finasteride in suppressing DHT and controlling hair loss.
Usage: Typically, 0.5 mg orally once daily.
Side effects: Similar to Finasteride, including possible sexual dysfunction, breast tenderness, and liver function impairment. The risk of side effects may be slightly higher with Dutasteride due to its broader enzyme inhibition.
Time to effectiveness: Results may appear within 3 to 6 months, and some users may experience results more quickly compared to Finasteride.
3. Minoxidil (Oral)
Brand names: Loniten, etc.
Features: Initially developed as a blood pressure medication, Minoxidil has a side effect of promoting hair growth. It works by dilating blood vessels and increasing blood flow to the scalp, which stimulates hair follicles. While typically used as a topical solution, oral Minoxidil is also prescribed for AGA.
Effects:
Promotes hair regrowth: Oral Minoxidil can offer more potent hair regrowth effects compared to topical applications. It promotes follicle health and increases hair density.
Usage: Depending on the patient’s needs, typically 2.5 mg to 5 mg taken orally once daily.
Side effects: Can include lower blood pressure, dizziness, palpitations, and increased body hair growth (hypertrichosis). Some users may experience fluid retention or weight gain.
Time to effectiveness: Hair regrowth usually becomes noticeable within 2 to 3 months, with significant results by the 6-month mark.
4. Other Oral Treatments
Spironolactone:
Features: Originally a diuretic, Spironolactone has anti-androgen effects, making it useful for AGA by reducing the impact of male hormones like DHT.
Effects: Reduces DHT’s effect on hair follicles, slowing hair loss.
Side effects: Hormonal side effects such as sexual dysfunction, breast enlargement, and electrolyte imbalances.
Flutamide:
Features: Used in prostate cancer treatment, Flutamide is an androgen receptor antagonist that blocks male hormones from affecting hair follicles.
Effects: Can slow hair loss by blocking DHT’s effects but is less commonly used due to potential risks.
Side effects: Can include sexual dysfunction and liver toxicity.
